I would like to acquaint you with our health policies. Sometime during the school year vision, hearing, blood pressure and scoliosis screening will be done. Students in K-4 will have height and weight monitored each year. Your implied consent to perform these screenings will be given unless a written objection is given to the school nurse. Please make note of the following: All medication that is to be administered by school personnel must be received in the container in which it was dispensed by the prescribing physician, licensed pharmacist, or in the case of over the counter medication, the original container All medication must be labeled with the student's name, dosage and the name of the medication. All medication must be brought to the school by the parent. The "Oral Medication Request" form must be signed by the parent/guardian and, if prescription medication, by the doctor. In the morning if your child has fever, pain, rash or an eye that is red with drainage, do not send him/ her to school. Old wounds that have become infected are not treated in school. Your doctor should check these conditions. The school nurse will administer first aid to those students who become ill or receive injuries in school. In case of serious illness or injury to your child and his/ her need for medical treatment, it is very important that the school know whom to contact and have your permission to take proper action. I, therefore urge you to complete the Emergency Medical Authorization and return it to the school within one week. Sincerely, |
